Making a referral
Referring a patient to Weldmar Hospicecare means they will be allocated a Weldmar Community Nurse or Nurse Specialist, who will then be able to arrange the relevant care and services for the patient and their loved ones.

Our Services
Weldmar Community Nurse and Nurse Specialists
Our community team operates across the north, south, and west of Dorset, and will visit patients in their own home to assess their palliative care needs and requirements, and the best way forward, including for their loved ones. The Weldmar Community Nurse will make internal referrals to any of our other services, as listed below. Find out more about our community nursing team.
Specialist Physiotherapy and Occupational Therapy
We have a team of Allied Health Professionals who work with patients based at our Inpatient Unit, or at home. Their aim is to allow patients to maintain their independence for as long as possible, and enable wellbeing and management of symptoms. Find out more about our therapies.
Patient and Family Support Services
We have recently introduced Wellbeing Sessions as part of this service, which have replaced what was formerly Day Services. In these sessions, people who are in our care – be they patients, carers, or those going through bereavement – can join us for tailored activities such as arts and crafts, gardening, traditional coffee mornings, and even a choir.
Looking after the carers and loved ones of our patients is very important to us, and our team provides counselling and bereavement support, complementary therapies, specialist palliative care social workers, and spiritual care. Find out more about our Patient and Family Support Services.
Inpatient Unit
Our Inpatient Unit is located in Dorchester, providing symptom control and pain management for more acute cases, as well as end of life care. Our team consists of specialty doctors (who can also make home visits), nurses, health care assistants, and therapists. We have a mix of two bedded bays, single rooms, and a family room. Weldmar at Home
This service launched in May 2021 and provides a rapid response solution for patients with advanced symptoms who want to die in the comfort of their own home. Weldmar Hospicecare Health Care Assistants are based across the county. Find out more about Weldmar at Home.
