Our Specialist Hospice at Home Service has a flexible approach to daily patient needs and a commitment to providing an at-home service that truly works for the individual. As a Weldmar Community Nurse, you’ll work within a multi-skilled, agile team dedicated to providing responsive, evidence based palliative care. This role is unique because it offers you the time to properly assess, make a difference, and build relationships with those you care for.
You’ll make a difference by:
- Using your experience to conduct ongoing assessments and deliver high-quality, holistic palliative care.
- Working alongside clinical specialists, other Weldmar services, and external agencies to ensure care is seamless and patient-centered.
- Providing clear direction, coaching, and line management to Health Care Assistants and Assistant Practitioners, fostering a culture of accountability and professional growth.
- Supporting the Community Nurse Specialist (Hub Lead) in creating a positive, high-performing environment for the whole team.
You’ll bring a solid nursing foundation with a passion for palliative and end-of-life care, and the ability to manage complex cases and provide clear advice to patients, carers, and the wider team. You’ll also need proven skills in line management, coaching, and supporting the development of colleagues
