Specialist Hospice at Home

A patient’s first contact with Weldmar Hospicecare will normally be with their locally based Specialist Hospice at Home hub. We have hubs in the north, south, and west of Dorset, serving the communities in towns, villages, and hamlets around them.

Each team comprises of a Weldmar Community Nurse Specialist, Weldmar Community Nurses, Assistant Practitioners, and Health Care Assistants, providing care and support in the comfort of patients’ own homes, or in care homes or community hospitals, throughout their journey with us. They can refer patients and their loved ones into other Weldmar services, and liaise directly with other health professionals such as GPs.
